The Licensed Clinical Social Worker (LCSW) is a vital part of our interdisciplinary team, providing telehealth and in-home behavioral health services to our members. This is a unique opportunity to build strong, personal relationships with a diverse patient population and make a real difference by providing comprehensive care that goes beyond a traditional clinical setting.
As a Licensed Clinical Social Worker (LCSW), you will:
- Conduct comprehensive behavioral health assessments and provide care primarily through telehealth, with occasional in-home visits.
- Create personalized care plans by collaborating with our interdisciplinary team to connect members with essential medical, psychiatric, and social services.
- Act as a member advocate, helping members and their caregivers navigate community resources, entitlement programs, and complex healthcare decisions.
- Offer mental health support by providing vital psychotherapy services using evidence-based practices.
- Develop and implement crisis management plans to support our members during urgent situations.
You should get in touch if:
- You have a Master’s Degree in Social Work from an accredited program and possess a valid New Jersey or Georgia LCSW license.
- You are comfortable conducting field visits within your county and neighboring counties, with up to three field visit days per month.
- You have 3+ years of clinical experience in an inpatient or outpatient care setting.
- You have experience connecting individuals with community and social service resources, or the drive and curiosity to research and build those connections.
- You maintain a valid New Jersey driver’s license and have access to reliable personal transportation.
- You are bilingual in English and Spanish (preferred, but not required), with excellent verbal and written communication skills.

A reasonable estimate of the base salary range for this role is $85,000 to $90,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
