Our Clinical Team is a group of caring and passionate professionals dedicated to improving the lives of our members by conducting in-home visits using technology, including telehealth. We’re looking for someone to join us in providing cutting edge patient-centered visits for our members.

Under the direction of the VP, Clinical Affairs, the Nurse Practitioner Manager will assume leadership accountability and responsibility for the Clinical Quality and Operations of the Clover Care Visit and Readmission Prevention Programs, inclusive of priority clinical areas such as medication adherence improvement. The Nurse Practitioner Manager will work with VP, Clinical Affairs, Medical Directors, Nurse Practitioners, Medical Assistant Leads, Medical Assistants, Social Workers, and other clinical and operational staff to ensure that Clover’s clinical programs provide high quality care and operate efficiently and effectively. This position will support the mission, vision, values and strategic initiatives of Clover and must demonstrate a commitment to quality service to members, community health care providers, and co-workers.

As a Manager, Nurse Practitioner, you will:

  • Support and performance manage your clinical teams including nurse practitioners, medical assistants, and registered nurses, where applicable.
  • Participate in and lead group and 1:1 meetings and educational sessions designed to advance program performance.
  • Coach, develop and manage the overall performance of clinical staff in the program.
  • Identify, coordinate, and oversee process improvement opportunities, including best practice clinical care protocols.
  • Maintain clinical competencies and provide direct care to Clover members.
  • Oversee clinical field supervision and performance management of clinical staff.
  • Ensure program reporting, data collection and required documentation is completed efficiently and effectively.
  • Develop new clinical initiatives to support Clover clinical and performance goals.
  • Provide excellent care management and care coordination services when providing direct clinical care.
  • Projects and additional responsibilities as assigned by leadership.

Success in this role looks like:

  • In your first 90 days, you will:
    • Develop a comprehensive understanding of Clover’s CCS program, workflows, and team dynamics, including collaborating closely with Medical Assistants and other staff.
    • Establish relationships with Nurse Practitioners (NPs) and Medical Assistants (MAs), gaining insights into their strengths, challenges, and feedback to implement early operational improvements.
  • After 6 months, you will:
    • Lead team meetings and one-on-one check-ins to foster professional development, resolve challenges, and ensure alignment with company goals.
    • Establish a collaborative and supportive team culture through effective communication
  • Future success in this role includes:
    • Driving the team to consistently exceed performance benchmarks, ensuring the delivery of exceptional patient care and alignment with organizational goals.
    • Develop and implement innovative best practices to elevate the CCS program, setting a standard for excellence

You should get in touch if:

  • You have a BSN degree and Master's in Advanced Clinical Practice Nursing (or post-Master's NP certificate), required.
  • You have a valid New Jersey nursing license, required.
  • You have a BLS and ACLS at or within 30 days of hire, required.
  • You have a valid New Jersey driver's license and reliable personal transportation to serve our members in their homes, required.
  • You have a passion for home healthcare, backed by 2+ years of experience (3-5 years is a plus!)
  • You have leadership experience guiding nursing teams and allied health providers (1-2 years minimum, 3-5 years ideal).
  • Been employed by Clover for a minimum of 4 Years
  • Strong computer literacy and EMR experience (familiarity with AthenaNet is a bonus).
  • You have care management certification, preferred.
  • You have geriatrics certification, preferred.
  • You have excellent communication skills in English, with Spanish proficiency being highly valued.
  • You have excellent interpersonal skills and an ability to communicate effectively with patients and colleagues alike.
  • You are comfortable walking up at least 3 flights of stairs and carrying up to 40 lbs of equipment, as some residences may not have elevators.
  • You are comfortable working with patients of diverse social, ethnic, and cultural backgrounds.

A reasonable estimate of the base salary range for this role is $170,000 to $180,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
